Antimalarials are drugs which are used forprophylaxis, treatment & prevention of Malaria. They are used for treatmentof Malaria in individuals with suspected or confirmed infection and forprevention of infection in individuals visiting a malaria-endemic region whohave no immunity (prophylaxis).
Dihydroqinghaosu is the active metaboliteto which artemesinin is reduced. It is the most effective artemesinin compoundand the least stable. It has a strong blood schizonticidal action and reducesgametocyte transmission. It is used for therapeutic treatment of cases ofresistant and uncomplicated P. falciparum. 4 mg/kg doses are recommended on thefirst day of therapy followed by 2 mg/kg for 6 days.
Pipreriquine is an antimalarial compoundbelonging to the 4-aminoquinolines. It is a highly liphophilic. PiperiquinePhosphate (PQP) interferes with physiological function of the food vacuolemembrane of the trophozoites leading to autophagocytosis of the parasites. Ithas no marked effect on the ring forms, immature or mature schizonts and themale of female gametocytes.
Trimethoprim acts by interfering with theaction of bacterial dihydrofolate reductase, inhibiting synthesis oftetrahydrofolic acid. Tetrahydrofolic acid is an essential precursor in the denovo synthesis of the intermediate Thymidine monophosphate (dTMP), precursor of DNA metabolite Thymidine triphosphate. Bacteria are unable to take up folicacid from the environment (i.e. the infection host) and are thus dependent ontheir own de novo synthesis. Inhibition of the enzyme starves the bacteria ofnucleotides necessary for DNA replication causing, in certain circumstances,cell lethality due to thymineless death.
